MAINTAINING MY STANDARD OF CARE

Beginning March 20, 2021 my practice will change to a membership model for all patients for whom I bill insurance. This is intended to keep my practice vibrant and allow me to provide the kind of holistic personalized medical care I've been delivering since 2002: 
  • Longer, more thorough office visits and physical exams
  • Readily available acute and urgent care visits, often same day or within a day or two at most
  • Punctual, courteous and efficient office services from friendly and available staff
  • Access to us through email and the Elation portal

Many of these benefits cannot be billed to insurance. If you'd like to read a more detailed description of this model and why I chose it, please read the detailed Membership Announcement Letter.
HOW IT WORKS
Every insured patient “joins” the practice by paying the annual fee of $360. Families of three or more people in the same household who are patients (of whom one is under age 25) pay $900. My goal is to have all current patient memberships set up by May 31, 2021.

If you're insured:
  • Please fill out the Membership Contribution Agreement and return it with your payment to Olivia at the office
  • This is a non-reimbursable, out-of-pocket expense
  • Your membership will renew the first of the month one year from your first contribution
  • I will continue to submit claims to insurance as usual
  • If joining the membership causes financial hardship, I am happy to discuss options on an individual basis
If you are self-pay or non-insured:
  • I will continue to offer a discounted fee to be paid at the time of service as well as a superbill you can submit for possible out of network insurance reimbursement
  • You are welcome to make a contribution if you feel so moved

​APPOINTMENTS & CANCELLATIONS
  • Appointments are scheduled by calling the front desk: ​206-906-9786
  • There is currently no online scheduling option.
  • I respect your time and almost always begin appointments on time. If you are more than 15 minutes late for your appointment you will need to reschedule.
  • ​If you need to cancel or reschedule your appointment please give 24 hours notice.
  • Cancellations of less than 24 hours or "No Shows" will be billed at $100.00 for 30 minute visits and $200.00 for physicals or longer appointments. These fees are not covered by your insurance and must be paid before another visit is scheduled.
  • Office appointment lengths:
    • New patient and annual physical exams: 60 minutes
    • Follow-up appointments: 20-40 minutes (depending on complexity)
  • Phone consultations are possible at my discretion. Fee is $70.00/15 minutes.​
  • Telehealth appointments can be billed to insurance. Self-pay fee $140.00/30 min​
AFTER HOURS / ON CALL
  • After hours phone triage advice is available 7 days/week when the clinic is closed. I share call responsibilities with 3 other physicians.
  • Call 206-783-1156 and follow prompts. We typically respond within 30 minutes.
  • If you don’t hear from us then please try again.

INSURANCE
  • I currently have contracts with these insurance companies and will bill them for my services as a courtesy. Be sure to check with your plan prior to an appointment to see if I am on that plan and if they reimburse for my services. I am an in-network provider for:
          - Regence​          - Premera      - Lifewise
          - First Choice Network          - Aetna
  • I welcome self-pay patients and offer them a discounted fee as well as a superbill with appropriate codes for those seeking reimbursement for out-of-network providers.
  • I am not a Medicare provider, nor am I contracted with Labor & Industries. I do not do disability work ups. PIP coverage is possible for established patients only.​​ 
​BILLING
  • I am exclusively a fee-for-service practice; please grant me the courtesy of paying your bills when you receive them. Repeatedly mailing invoices unnecessarily adds to my overhead.
